Please fill out this entire application if you wish to be considered for the 2011 Summer @ CENS Undergraduate Scholars Program.

Preparing to Apply:
You must first register to begin the application process.

A complete application must include the following:

1. A completed Online Student Application Form.

   Upon completion of a submitted form you will receive an email confirmation verifying the submission of your application form.

   If you are having difficulties with the online application system, you can contact Wesley Uehara (wuehara@cens.ucla.edu) or (webmaster@cens.ucla.edu) with the following information:

   1. Which browser you were using and what platform (OS X, Windows XP)
   2. The error message that you received (cut & paste into your email message)
   3. What you were doing at the time, on which page etc. (cut & paste the url into your email message)

2. An official copy of your most current Undergraduate Transcripts. (Please include the most recent transcripts from all institutions you have attended.) Please note that these institutions may have a processing period.

3. Your Personal Statement.

   Your statement should describe the following within one to two pages:
   

   • Your research interests
   • Your previous research experience and how this program might prepare you for the future
   • Your reasons for wanting to participate in this research opportunity
   • If you believe that your academic record does not accurately reflect your potential, please explain (briefly).

4. Your Resume.

5. One Letter of Recommendation.

   One letter of recommendation is required. A letter from a faculty member is preferred, but other professional references will be accepted. Please download and print the letter of recommendation form, fill out your portion and give it to your recommender.

   Request that your recommender mail the completed form and their letter directly to the address provided on the form. Your letter writer can also choose to send (email) a PDF of their letter and the signed form to Wesley Uehara at wuehara@cens.ucla.edu.
